Student Assessments

 

 

The system provides for a teacher to be able to view a student’s record and add a new fitness assessment to that student. To do this, the teacher opens his/her web browser to the Internet address given for the school's LiftAmerica web site.

 

On the school page there is a list of classes. Click the class where the student is enrolled. See example below.

 

 

Selecting the class name will bring up the Teacher’s Page, where the Select A Student link may be accessed. This link retrieves a list of all students who are enrolled in the class.

 

 

To select a student, click on the Select link. The student’s record summary will be returned (see the example below).  Each of the information sections can be viewed by clicking on the link in the menu navigation to the left or on the section heading itself.

 

 

 

 

 

New Fitness Assessment

 

Entering a new fitness assessment consists of completing two (2) pages containing just three (3) sections - Personal Information, Contact Information and New Fitness Assessment.

 

Start out by clicking on the New Fitness Assessment item at the bottom of the record summary menu navigation for the selected student. The first page of the assessment form will automatically be displayed.

 

The two (2) sections contained on the first page should have been previously filled out at the time of enrollment. You will then be asked to verify its accuracy. Continue by pressing the Next  button.

 

The second page contains sixteen (16) mandatory, asterisked  items and several optional fields, as shown below. If an asterisked field is not filled in or is filled in incorrectly, an error icon  will appear next to the field box. The screen will not save until all fields with error icons have been corrected.

 

<  Date Entered – The current date is already shown in the field, however if you need to

make changes, you many use any format for date entry. Entries such as:

                        Jan 1, 2001

                        January 1, 01

                        1/1/2001

                        01/01/01

                        may be typed and will be standardized by the system.

            <  Date Assessment Completed – As above, the current date is displayed in the field, but

may be changed using any date format.

<  Height – Enter the height in feet and inches

<  Weight – Enter the weight in pounds between 40 and 750

            <  Shoulder Flexibility – Enter a number between –25.0 and 20.0

<  Blood Pressure (Systolic) - Enter a whole number between 70 and 220

<  Blood Pressure (Diastolic) - Enter a whole number between 40 and 180

<  Resting Heart Rate - Enter a whole number between 35 and 100

            <  Body Composition - Enter a number between 2.0 and 50.0

<  Step Test - Enter whole numbers between 45 and 200 in each of the Step Test fields

<  Hip Flexibility – Enter a number between –15.0 and 150.0

<  Seated Chest Press - Enter whole number between 10 and 300

<  Seated Leg Press - Enter a whole number between 15 and 500

< Restrictions - Enter any physical restriction notes in this space

            < Grade in school – Enter the number of the grade year

 

When each of these sections has been filled in, click the Save  button and the new assessment will be complete.
